The Policies of Our Practice

Before your visit, we would like to tell you more about Macke Dental Care in regard to our patient policies and business practices. Our aim — always — is to meet all of your dental needs in ways that will make you feel very confident and comfortable with us.

Your First Visit

Nancy Queen is our office manager, scheduler, and greeter. As a new patient, she will reserve one hour for your first appointment, allowing you quality time to become acquainted with Dr. Macke or Dr. Rose. The first visit also includes a review of your medical and dental history, a thorough oral exam, an evaluation of your oral health, and then a discussion of your concerns and our recommendations for treatment. We will always take time to listen to you, and want you to be fully informed. Be assured, we will give you honest answers and never sell you unnecessary procedures.  

Your first visit establishes the foundation for our relationship, so we will need to get important background information from you, including your medical and dental history. You can pull up the Patient Information Form on the link below. We also ask you to sign a HIPAA (Health Insurance Portability & Accounting Act) compliance form that acknowledges we have advised you that we do not give out private information. Please fill out these forms, fax them, or bring them in at the time of your appointment. Or, Nancy will give you the forms to complete in the office; in this case, she suggests that you arrive 15 minutes early for your appointment.

You can bring us your x-rays taken previously at another dental office and we’ll gladly review them. However, we will take new x-rays in order to give us current information on your oral health and give you a proper diagnosis.

At the end of your first appointment, the doctor will recommend a comprehensive treatment plan for you, discuss it with you, and answer any questions you have. At that time, we will set up a first-phase visit for you with one of our hygienists for a professional cleaning and/or periodontal services.

Insurance Coverage

We accept most insurance plans that allow you to choose your dentist. Please confirm your dental coverage with your insurance company and bring your insurance information and card with you when you make your first visit.

We feel it’s important for you to understand how dental insurance works.

•    Unlike with medical insurance, dental insurance companies put limits on dental expenses -- usually $1,000 or $1,500 per year.
•    To determine how much they will reimburse for services, insurance companies survey dental practices in geographical areas and calculate the average fee within each area. Then, customarily, they set their reimbursements at 80 percent of the average, which means they cover only part of treatments.
•    The surveys conducted by the insurance companies include discount dental clinics and managed care facilities, which drastically reduce their fees and bring down the average. Because of this, most doctors in private practice charge fees that insurance companies define as “higher than usual and customary.”
•    Every plan is different. Your benefits are based on the contract your company negotiated with your insurance carrier. If you have questions about your plan, the best way to get answers is to contact your company’s benefits department.

Nancy will work with your insurance company to maximize your benefit. She will be glad to file your claim for you and bill your carrier for reimbursement for your treatment. However, at the time of your visit, you will be responsible for paying the portion of the bill not covered by insurance.

Financial Matters

Our primary mission is to deliver the best and most comprehensive dental care available. An important part of this mission is making the cost of optimal care as easy and manageable as possible for you.

For your convenience, we accept cash, check, MasterCard, and Visa payments. And, we’ll be glad to discuss the fee with you before your appointment.

We also participate in CareCredit, a third-party group that offers no-interest financing for dental procedures, along with payment terms that fit patients’ needs. We have their information and applications in our office. Just ask Nancy, or you also can obtain them online.


To schedule an appointment, please call us at 770.993.0265 during our regular office hours, or leave a voicemail message and Nancy will call you back as soon as possible.  She will find an appointment time that fits your needs and schedule.

For patients under the age of 18, we require a written authorization before treatment by his/her parent or legal guardian.

We understand that unforeseen things come up and you may have to cancel an appointment. If so, we ask you to give us 24 hours’ notice. Patients are charged a fee of $25 if they miss or cancel more than one time in a calendar year without a 24-hour notice.

We are open to serve you Monday through Wednesday, 8 a.m. to 5:30 p.m., and Thursday, 8 a.m. to 12 noon. We are closed on Fridays.
